Before booking your Medical Hypnosis Session, you must read this General Disclaimer. Booking your session indicates that you have read and agreed to the terms and conditions summarized below.

Medical Hypnosis is committed to offering consistent quality and professional hypnotherapy. This is not a crisis centre. If you are seriously contemplating suicide or believe that you or others close to you may be in danger, please immediately contact your nearest crisis center, physician or hospital.. Medical Hypnosis Office offers services to all people over 18.  Those under 18 must have a written consent and met the Hypnotherapist in person prior to session. 

Your session with the Hypnotherapist is not intended to and may not be used as a substitute for medical diagnosis or treatment. You should seek the advice of your physician or of a qualified health provider prior to starting any treatment or if you have any questions related to your mental or physical health, physical fitness, or medical conditions. The Medical Hypnosis Office is not engaged in the provision or practice of medical, nursing, or healthcare advice or services in any jurisdiction. The Hypnotherapist acts as a guide for you as an individual to communication your own messages and desires via the form of a hypnotic relaxation technique.

MEDICAL HYPNOSIS CENTER EARNESTLY RECOMMENDS THAT YOU FIRST CONSULT YOUR PHYSICIAN OR MENTAL HEALTH COUNSELOR PRIOR TO MAKING YOUR APPOINTMENT. ALSO, THAT YOU DO NOT STOP OR ALTER ANY TREATMENT YOU ARE CURRENTLY RECEIVING WITHOUT PRIOR CONSENT FROM YOUR PHYSICIAN. IF YOU HAVE A MEDICAL EMERGENCY, DIAL 999 (911) IMMEDIATELY.  MEDICAL HYPNOSIS CENTER IS NOT RESPONSIBLE FOR AND WILL NOT BE LIABLE TO YOU OR ANYONE ELSE FOR ANY ACTION OR DECISION TAKEN IN RELIANCE UPON THE INFORMATION CONTAINED IN YOUR SESSION. 
 

Privacy Statement

Your privacy is important to the Medical Hypnosis Center.. At The Medical Hypnosis Office we want you to feel as comfortable as possible visiting our offices. This Privacy Statement will tell you what information we collect, how we protect it, and what choices you have about how that information is used. We urge you to read our Privacy Statement so that you will understand both our commitment to you and your privacy, and how you can participate in that commitment.

Please remember that any information (including personal and medical information) that you reveal in a public forum never be used for unauthorized purposes.

Information We Collect:
If you use our services we require the use of a credit card, we will use the appropriate information to charge your credit card.  You information is never shared by other parties and we do not retain your credit card number unless you request us to do so.  Ergo we ask you for your card number 24 hours prior to each session.

Disclosure of Your Personal Information:
Medical Hypnosis Center will never disclose any information it gathers from you that could be used to identify or contact you ("Personal Information"). Any Personal Information provided will be treated in accordance with the terms of this Privacy Statement, unless you are otherwise notified. Medical Hypnosis Center will never intentionally disclose any Personal Information about you as an individual. We will never give your name, address, telephone number, credit card number, or e-mail address to anyone ever without your permission.
In certain circumstances we may be required to release Personal Information to third parties, such as to comply with a law, regulation, search warrant, subpoena or court order, or in special cases, such as a physical threat to you or to others.
Medical Hypnosis Center will video tape each session.  As a patient you agree to this tapping.  However these tapes are for the purpose of the Hypnotherapist anaylaization of you session only.  And are kept for one year off site in a fire proof vault.  These videos are not available for patient.


Your Privacy Choices:
When you complete your registration, you may be given the option of receiving informational and/or promotional emails from Medical Hypnosis Center.. You may elect not to receive these emails.
